Perimenopause supplements
Perimenopause is the most heavily marketed moment in a woman's health, and most of what is sold into it does very little. This is what the evidence supports, what it does not, and what each option costs you a day.
The context.
Perimenopause typically runs for four to eight years before periods stop, and the symptoms - broken sleep, mood volatility, brain fog, joint aches, flushes, changing cycles - arrive gradually enough that most women spend a long time trying to solve them one at a time. That is exactly the gap supplement marketing occupies.
The honest position is that no supplement treats perimenopause, because perimenopause is a hormonal transition, not a nutrient deficiency. What supplements can do is support the things that make the transition harder: poor sleep, low magnesium or vitamin D status, falling muscle mass and inadequate protein.

What to look for.
- The elemental amount, not the compound weight
- 1,000mg of magnesium bisglycinate may be 130mg of actual magnesium. The useful number is always the elemental figure.
- Doses that match the trials
- Black cohosh was studied at 40mg standardised extract. A blend containing 5mg of it is decoration.
- A short ingredient list
- A product with 22 botanicals almost certainly contains a token amount of each. Three ingredients at real doses beats twenty at trace levels.
- Cost per day, not cost per pack
- A 30-day pot at £24 and a 60-day pot at £38 are not the same purchase. We calculate this for you in the table.
What to ignore.
- 'Hormone balancing'
- This phrase has no clinical meaning. Nothing in a capsule adjusts your oestrogen and progesterone in a targeted way.
- Natural alternatives to HRT
- There is no over-the-counter product that replicates what body-identical hormone therapy does, including for bone protection.
- Proprietary blends
- If the label gives one total weight for a blend rather than per-ingredient amounts, you cannot know what you are taking.
The ingredients, one by one.
Magnesium glycinate
Genuinely useful if you are low in magnesium or sleeping badly. Not a sedative, and not a cure for anxiety.
Black cohosh
Worth a supervised trial if you cannot or will not take HRT. Stop at once if you develop any sign of liver trouble.
Red clover
Weak and inconsistent evidence. If you want to try a plant oestrogen, dietary soy has better data.
Vitamin D and K2
Vitamin D is a genuine UK public health recommendation. K2 is reasonable, optional and oversold.
Collagen
Real but small skin effects. If your total protein intake is low, plain protein and vitamin C matter more.
Creatine
One of the few supplements that genuinely works, provided you are actually training. On its own it does very little.
Omega-3
Worth taking if you rarely eat oily fish, and genuinely important in pregnancy. Not a mood treatment on its own.
When a supplement is not the answer
If hot flushes are waking you nightly, if your mood has changed in a way that frightens you, if bleeding has become heavy or unpredictable, or if sex has become painful, those are clinical problems with clinical answers. HRT, vaginal oestrogen and non-hormonal prescribed options all have far better evidence than anything on this page. A supplement taken instead of that conversation costs you time you do not need to lose.

What is the best supplement for perimenopause?
There is no single best one. Magnesium has the most support for sleep and cramps, vitamin D matters for bone health in the UK, and protein plus creatine alongside strength training protects the muscle you lose fastest in this decade.
Do perimenopause supplements actually work?
Some help specific symptoms. None treat the hormonal transition itself. Products claiming to balance hormones or replace HRT are overstating what a supplement can do.
Should I take supplements or HRT?
They answer different questions. HRT treats the oestrogen decline and has the strongest evidence for flushes, sleep, mood and bone protection. Supplements address nutrient gaps and general resilience. Many women reasonably do both.
How much should perimenopause supplements cost?
Well-formulated single ingredients cost roughly 15p to 60p a day. Multi-botanical menopause blends often cost £1 to £2 a day for smaller doses of each ingredient.
