Red clover
A plant oestrogen with a long marketing history and an underwhelming trial record. Worth understanding before you spend money on it.

What the evidence actually shows
Red clover isoflavones bind weakly to oestrogen receptors, which is the basis for the hot flush claim. Randomised trials have produced mixed and largely modest results; systematic reviews generally conclude the evidence does not support routine use, while noting a possible small effect in some studies.
Dietary soy isoflavones have somewhat better data than red clover extracts, and come with the general benefits of eating soy foods. If the goal is a plant-oestrogen approach, food is the more sensible starting point.
NICE guidance on menopause notes that some women find isoflavones helpful, while cautioning that preparations vary in composition and that safety data is limited. That is a fair summary of where things stand.
The breast cancer question
Because isoflavones have oestrogenic activity, women with a history of breast cancer, or taking tamoxifen or an aromatase inhibitor, are routinely advised not to take concentrated isoflavone supplements without specialist agreement. The theoretical concern has not been resolved by good long-term data, and the stakes are high enough that caution is the right default.
This is separate from dietary soy, which has not been shown to increase breast cancer risk and is generally considered safe as a food.

Who should avoid it.
- You have or have had a hormone-sensitive cancer, unless your specialist agrees
- You are taking tamoxifen or an aromatase inhibitor
- You are pregnant or breastfeeding
- You have unexplained vaginal bleeding - get that investigated first

The trial evidence is inconsistent and any effect appears small. Some women report benefit, but pooled analyses do not support routine use.
Is red clover safe with breast cancer?
Concentrated isoflavone supplements are generally not recommended for women with a history of hormone-sensitive breast cancer. Speak to your oncology team before taking anything of this kind.
Red clover or soy isoflavones?
Dietary soy has better evidence and a better safety record. If you want to try a plant-oestrogen approach, start with soy foods rather than a concentrated extract.
