Why this one needs a blood test first
Iron deficiency is genuinely common in menstruating women, and correcting it can transform energy, concentration and exercise tolerance. But the symptoms - tiredness, low mood, poor concentration - overlap with a dozen other things, including thyroid disease, perimenopause, sleep disruption and depression. Taking iron on a hunch means you might mask something else while fixing nothing.
Ferritin is the usual first test, with a full blood count. Ferritin rises with inflammation, so a normal-looking result in someone unwell can still hide deficiency. This is exactly the kind of nuance a clinician should interpret, not an algorithm on a supplement site.
Unexplained iron deficiency in a woman who is not menstruating heavily needs investigation. It can be the first sign of gut blood loss or coeliac disease. Supplementing without asking why is the mistake that delays diagnoses.
Getting more out of the dose you take
Absorption improves with vitamin C - a glass of orange juice or a vitamin C tablet with the dose is a genuinely useful trick. Tea, coffee, calcium and dairy all reduce absorption and should be separated by an hour or two.
Alternate-day dosing is now often preferred to daily dosing. Taking iron every day raises hepcidin, a hormone that blocks further absorption, so every-other-day dosing can deliver as much iron with fewer side effects. This is a change worth knowing about if you have struggled with iron in the past.
Gentler forms such as bisglycinate cause less constipation and nausea than ferrous sulfate, though they typically contain less elemental iron per tablet. If standard iron has been intolerable, this is a reasonable route to raise with your GP or pharmacist.

- Recheck bloods after around three months. Deficiency usually takes months to correct fully, and stores need replenishing after haemoglobin normalises.
What it will not do.
- It will not boost energy if your iron is already normal
- It will not fix fatigue caused by poor sleep, thyroid disease or perimenopause
- It will not work overnight - expect weeks, not days
Who should avoid it.
- You have haemochromatosis or any iron overload condition
- You have not been tested and have no clear reason to be deficient
- You are having repeated transfusions
Raise with a prescriber
- Reduces absorption of levothyroxine - separate by four hours
- Reduces absorption of some antibiotics and bisphosphonates - separate by two hours
- Calcium supplements, tea and coffee reduce iron absorption

Should I take iron for tiredness?
Only after a blood test. Tiredness has many causes, and taking iron you do not need is both useless and potentially harmful.
What is the best time to take iron?
On an empty stomach with a source of vitamin C, avoiding tea, coffee and dairy for an hour either side. Alternate-day dosing is often better tolerated and absorbed than daily.
Do heavy periods cause iron deficiency?
They are the commonest cause in UK women. If your periods are heavy enough to affect your iron levels, that is worth treating in its own right, not just supplementing around.
How long until iron tablets work?
Energy often improves within a few weeks, but replenishing stores takes around three months. Bloods are usually rechecked at that point.
